Coding regulation ผู้ให้รหัสสามารถให้รหัสได้เฉพาะการผ่าตัดหรือหัตถการที่แพทย์บันทึกใน discharge summary ถ้าเห็นว่าแพทย์บันทึกการผ่าตัดหรือหัตถการไม่ครบ ต้องปรึกษาแพทย์และขอให้บันทึกเพิ่มให้ครบ ถ้าแพทย์บันทึกผิดหรือบันทึกไม่ ละเอียดย่อมมีโอกาสได้รหัสผิดสูง เพราะผู้ให้รหัสไม่ใช่แพทย์
