ให้ความสำคัญกับชีวิต ไม่มีเจตนาที่จะเร่งหรือยีดการตาย Palliative care (WHO 1989) เป็นการดูแลชีวิตผู้ป่วยแบบองค์รวม (the active total care of patients & family) ในผู้ป่วยระยะสุดท้าย (เป็นโรคที่รักษาไม่ได้ เป็นมากจนดำเนินชีวิตประจำวันได้ยาก และคาดว่าจะเสียชีวิตในเวลากันใกล้) เป้าหมาย เพื่อคุณภาพชีวิตของผู้ป่วยและครอบครัว ให้ความสำคัญกับชีวิต ไม่มีเจตนาที่จะเร่งหรือยีดการตาย
Palliative Care (WHO 2002) เป็นแนวทางการดูแลที่มุ่งพัฒนาคุณภาพชีวิตของผู้ป่วยและครอบครัว ซึ่งเผชิญกับโรคที่คุกคามชีวิต HIV, chronic medical, critical illness โดยให้การป้องกันและบรรเทาความทุกข์ทรมานแบบองค์รวม การวินิจฉัยผู้ที่ต้องการ palliative care แต่เนิ่นๆ การประเมินให้ครอบคลุมสมบูรณ์ทุกด้าน ดูแลความปวด และอาการทรมานอื่นๆ อย่างครบองค์รวม อาจร่วมกับเคมีบำบัด การฉายแสง และการตรวจทางห้องปฏิบัติการ เพื่อทำให้เข้าใจและรักษาภาวะแทรกซ้อนที่ทำให้เกิดการทรมาน

การบรรเทาอาการเป็นงานหลักใน palliative care ประเมินอาการทรมานและความสำคัญ โดยผู้ป่วย เรามีหน้าที่ช่วยหาสาเหตุที่แก้ไขได้ มักเป็นเหตุร่วมกัน (โรคที่เป็น ผลข้างเคียงการรักษา โรคที่พบร่วม) ให้การรักษาเฉพาะ ในเหตุที่แก้ไขได้ ให้การรักษาตามอาการ อะไรที่เคยได้ผล Complementary /Psychology /Rehabilitation Medications (morphine, sedation, anti-emetic) ใช้วิธีการที่ไม่รบกวนคุณภาพชีวิต (ตัดสินโดยผู้ป่วย)

DYSPNEA = Breathlessness = Unpleasant awareness of difficulty in breathing 55-70% in patients near the end of life More severe in last few weeks before death More common in CA lung, colorectal, breast May be related to cancer itself, previous treatment or concomitant problem e.g.COPD

Symptomatic Drug Treatment Morphine First choice agent for symptom control Reduces sensation of breathlessness Reduces respiratory drive Doses required usually lower than that used for pain Dyspnea directly antogonizes the sedative and respiratory depression of opioids (alert, normal breath with high doses of opioids)
