ผู้ป่วยเพศหญิง อายุ 46 ปี

CC : ฟันกรามน้อยด้านบนที่อุดไปเสียวและมีเศษอาหารมาติด HPI : ตอนแรกผู้ป่วยมีอาการเสียวฟันเมื่อดื่มน้ำเย็นและทานอาหารหวาน เมื่อหยุดทานอาการจะหายไปได้เอง มีอาการมา 2-3 เดือน จึงมาพบ นทพ. เพื่ออุดฟัน หลังจากอุดฟันไป 1 สัปดาห์ ผู้ป่วยมีอาการเสียวฟันมากกว่าเดิม

Effect of patient : ทำให้เคี้ยวอาหารได้ลำบาก Patient’s belief : ยังอุดฟันได้ไม่แน่น จึงมีช่องว่าเหลืออยู่ทำให้เศษอาหารเข้าไปติดได้และเสียวฟัน

Past & Current Systemic Medical Conditions : รักษาอาการทางจิตเวช เนื่องจากมีความเครียด Family history : No Psychosocial history & Environmental factor : ตอนนี้อาศัยอยู่กับสามี ซึ่งทำงานอยู่ที่ศูนย์ป้องกันสาธารณภัย ส่วนผู้ป่วยมีสวนยาง มีปัญหาด้านค่าใช้จ่ายเนื่องจากต้องส่งลูกเรียน 2 คน

Clinical finding at area of chief complaint 15OD amalgam filling, sensation to explration and air blow, positive to cold test

Other clinical findings 11 discoloration tooth, non response to EPT test at 80, 11M arrested caries 12Pa, 21M, 22Pa, 28O dental caries, without symptoms 17 O amalgam filling, 17D large dental caries exposed pulp ,positive to exploration and percussion 12 Pa ,21M ,22Pa ,28O dental caries ,patient has no any symptoms. 25OM amalgam filling present secondary caries patient has no any symptoms 13B ,14B ,16B abfraction, no sensation on exploration and air blow, patient has no any symptoms
