นายวิจิตร เงินมาก อายุ 30 ปี ได้รับการวินิจฉัยว่าเป็น ไข้หวัดใหญ่ ผู้ป่วยร้องขอให้ แพทย์สั่งยาปฏิชีวนะให้ ท่านจะตัดสินใจอย่างไร

นายยอดชาย รักไทย อายุ 36 ปี เพิ่งทราบ ผลการตรวจเลือด HIV ให้ ผลบวก นายยอดชายขอร้องไม่ให้ท่าน บอกผลการ ตรวจเลือดแก่ภรรยา อ้างว่ายัง ไม่พร้อม

นายช่วง หนูสง อายุ 68 ปี อาชีพทำสวน มี ประวัติสูบบุหรี่ วันละ 2 ซองมาตั้งแต่อายุ ประมาณ 20 ปี มีอาการไอ หอบ และเหนื่อย ง่าย ได้รับการวินิจฉัยว่าเป็น COPD มา ประมาณ 1 ปี เคยถูกรับตัวไว้ในโรงพยาบาล และถูกใส่เครื่องช่วยหายใจมาแล้ว 4 ครั้ง เนื่องจาก respiratory failure อยู่โรง พยาบาลครั้งละประมาณ 1 เดือน ขณะนี้ functional class III-IV

ครั้งนี้ถูกรับไว้ด้วยเรื่อง severe pneumonia และ respiratory failure ไม่รู้สึกตัว อยู่ โรงพยาบาลมาประมาณ 4 สัปดาห์ ได้รับการ รักษาด้วย antibiotics เปลี่ยนมาหลายชนิด ครั้งสุดท้ายได้ ciprofloxacin, bronchodilator และ supportive ventilator ผู้ป่วยยังไม่รู้สึก ตัว และยังมี moderate hypoxemia และ hypercapnia
