วัตถุประสงค์. เพื่อศึกษาผลของการใช้ High-power, pain threshold, วัตถุประสงค์ เพื่อศึกษาผลของการใช้ High-power, pain threshold, static ultrasound technique ใน Acute active myofascial trigger points ของ Upper trapezius ทีมีผล ต่อ Pain และ Active cervical lateral bending Methods ศึกษาในผู้ป่วยที่มีอาการปวดบริเวณ Upper trapezius ข้างเดียว จำนวน 72 คน (ชาย 25 คน หญิง 47 คน) Inclusion 1. มี active myofascial trigger points ของ Upper trapezius 1 ข้างอย่างน้อย 1 จุด 2. มีอาการปวดตั้งแต่ 0-2 สัปดาห์ 3. อายุระหว่าง 18-60 ปี

4. Primary MFPS 5. ไม่เคยรับการรักษาทางกายภาพบำบัดหรือยาเพื่อลดปวดมาก่อน จากนั้นแบ่งผู้ป่วยออกเป็น 2 กลุ่ม โดยการสุ่ม 1.Study group High-power, pain threshold, static ultrasound technique - Continuous wave mode , keep static on trigger point - Gradually increase intensity to the level of maximum pain for 4-5 sec. and reduce to half intensity level for 15 sec. Repeat 3 times

2. Control group Conventional, stroking, ultrasound technique - Stroking technique - Continuous wave mode - Intensity 1.5 W/cm2 5 min หลังจากนั้นผู้ป่วยทั้ง 2 กลุ่ม ให้ทำ Active stretching โดยการ เอียงศีรษะไปด้านตรงข้าที่ปวด (maximum voluntary contraction เป็นเวลา 30 วินาที ทำซ้ำ 5 ครั้ง) Main Outcome 1. VAS 2. Active ROM (Lateral bending of neck) 3. Length of treatment (number of therapy sessions)

Result -No statistically significant differences between the groups in age gender, baseline VAS score and ROM. -The mean number of session in gr.1 = 2.84 ± 1.4 และ gr.2 = 11.8 ± 2.47 (P < 0.05) - High statistically significant differences (P < 0.05) in mean VAS score and ROM of gr. 1 at the of first session - After complete the study ROM measurements of all patient show equal values but statistically significant differences (P < 0.05) between mean VAS score of gr. 1 after the last session.

Not to apply high power static ultrasound on trigger Discussion 1.Not to apply high power static ultrasound on trigger points in -vicinity of bony and neural structures 2.ผู้ป่วยจะบอกความรู้สึกไม่สุขสบาย( pain and burning) เมื่อ ultrasound ที่บริเวณ -periosteum -symphathetic chain -pheripheral nerve

3. not recommend apply for active trigger point of -facial and paraspinal m. -m. adjucent to nerve and bone structure 4.accururate diagnosis with knowledge of regional anatomy 5. good communication and concentration for both patient and therapist

Esenyel et al (2000) รายงาน การรักษา myofascial trigger points of upper trapezius m. โดยการใช้ combine neck stretching exs., conventional US และ trigger point injection ให้ผลไม่แตกต่างกัน Gam et al (1998) การรักษาด้วย conventional US และ sham ให้ผลไม่แตกต่างกัน ในการรักษา myofascial trigger points of neck and shoulder

1. John Z et al (j.pain 2008.04.009) รายงานว่า การใช้ low dose US สามารถกระตุ้น short term segmental antinociceptive effect on trigger ซึ่งสามารถนำไปใช้ในการรักษา muscoluskeletal pain ได้ โดยการเปรียบเทียบระหว่าง low dose US (10 นาที 1 MHz 12% duty cycle)และ sham US (10 นาที ) แล้ววัด pain pressure theshold 2. Srbely JZ and Dickey JP (Clin Rehabil.2007 May;21(5):411-7) Randomized controlled study ศึกษาผลของการใช้ Therapeutic US และ Low intensity US ที่มีต่อ pain sensitivity of myofascial trigger point ของ upper trapezius

ผลของการศึกษาพบว่าหลังจากการทำ Therapeutic US ค่า pain pressure threshold score เพิ่มขึ้น 14.2 % แต่หลังจากการทำ Low intensity US ค่า pain pressure threshold score ก่อนและหลัง ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ 3.Chuen et al (APMR 2002) ศึกษาผล(immediate effect)ของการใช้ Physical therapeutic modalities ใน ผู้ป่วย Cervical myofascial pain and trigger point sensitivity พบว่า การใช้ Therapeutic combination ระหว่าง 3.1 hot pack + active ROM+ stretching with spray และ hot pack + active ROM+ stretching with spray + TENS ให้ผลดีไม่แตกต่างกัน

3.2 ระหว่างการใช้ hot pack + active ROM+ interference current และ Myofascial pain release technique ให้ผลดีไม่แตกต่างกัน สรุปว่าทั้ง 4 technique ทำให้ pain sensitivity ลดลง และเพิ่ม cervical ROM 4.Hsieh et al (APMR.2007 May;86(5):397-403) ศึกษาผลการเปลี่ยนแปลง pain pressure theshold ของ secondary (satellite) myofascial trigger point (MTrPs) หลังการทำ Dry needling บริเวณ (primary) key active MTrPs ในผู้ป่วย bilateral shoulder pain and active MTrPs bilateral IFS โดยทำการ Random ในการทำ Dry needling ข้างใดข้างหนึ่ง

ผลของการศึกษาพบว่า ข้างที่ทำ Dry needling มีการลดลงของ pain pressure theshold และ pain intensity อย่างมีนัยสำคัญ ผู้วิจัยได้สรุปว่า Dry needling จะกระตุ้น inactive of a primary (key) MTrPs จะช่วยยับยั้ง active secondary MTrPs (refer pain zone)
