Head of mandibular condyle รูปไข่ อยู่เหนือต่อ mandibular neck มีความยาวเป็น 2 เท่าของความกว้าง long axis ตั้งฉากกับ plane ของ ramus lateral pole ยื่นเลยมาเล็กน้อย เป็นที่เกาะ ของ disc และ temporomandibular ligaments medial pole ยื่นเลยเข้ามามากกว่า เป็นที่เกาะของ disc sagittal crest แบ่ง medial slope กับ lateral slope

Functional part (pressure bearing surface) of condyle Articular surface ของ condyle Anterosuperior aspect, Medial surface Fibrocartilage ที่คลุมอยู่บน articular surface บริเวณนี้จะหนา

Neck แบนทาง anteroposterior ด้าน anteromedial มี Pterygoid fovea ด้าน posterior เรียบ มี lateral ligament มาเกาะที่ lateral และ posterior aspect

Mandibular fossa

S P T 2 1 medial lateral 3 S = squamous part P = petrous part T = tympanic part 1 = squamotympanic fissure 2 = petrosquamosal fissure 3 = petrotympanic fissure

Functional part (pressure bearing surface) of temporal bone 1. Posterior slope ไปจนถึง articular eminence 2. Preglenoid plane 3. Entoglenoid process รวมถึง medial glenoid plane Fibrocartilage ที่คลุมอยู่บน articular surface บริเวณนี้จะหนา

Articular disc Fibrous plate รูปไข่ ขอบหนา (anterior rim และ posterior rim) ยึดกับ capsule เกือบรอบ ยกเว้นด้าน medial และ lateral ที่ยึดกับ medial pole และ lateral pole ทำให้ disc กับ condyle เคลื่อนที่ไปด้วยกัน

ด้านหน้ายึดกับ capsule มี upper fibers ของ upper head of lateral pterygoid muscle มาเกาะทาง anteromedial ด้านหลังยึดกับ capsule ด้วย retrodiscal pad ซึ่งประกอบด้วย - upper layer ไปยึดกับ postglenoid lip & tubercle - lower layer ไปยึดกับด้านหลังของ condylar neck ส่วนที่รับแรงกดคือ anterior rim ของ disc

Fibrous capsule บริเวณ upper compartment จะหย่อน Dense fibrous tissue หุ้มรอบ joint มี upper fibers ของ upper head of lateral pterygoid muscle มาเกาะทาง anteromedial surface บริเวณ upper compartment จะหย่อน บริเวณ lower compartment จะตึงกว่า

Function of synovial fluid Synovial membrane - loose CT บุด้านในของ capsule - สร้าง synovial fluid Function of synovial fluid 1. Lubricate surface ลด friction 2. Source of nourishment for fibrocartilage 3. Phagocytose microorganism and tissue debris

Articular ligament 1. Temporomandibular ligaments มี 2 ชั้น 1.1 lateral ligament : prevent lateral dislocation of the joint 1.2 medial ligament : prevent excessive retrusive movement

Articular ligament 2. Accessory ligaments 2.1 Sphenomandibular ligament : ป้องกันการกระจายของยาชามาทาง medial 2.2 Stylomandibular ligament : limit excessive protrusive movement

Blood supply Anterior - posterior deep temporal artery - masseteric artery - lateral pterygoid artery Posterior - branch from superficial temporal artery - deep auricular artery - anterior tympanic artery - ascending pharyngeal artery

Nerve supply 1. Auriculotemporal nerve เลี้ยงทางด้าน posterior, medial, lateral 2. Masseteric & Deep temporal nerve เลี้ยงทางด้าน anterior
