29 การอักเสบติดเชื้อ บริเวณกระดูกใกล้ข้อ
Septic Arthritis การอักเสบติดเชื้อ บริเวณกระดูกใกล้ข้อ หนองที่เกิดจะไม่ผ่านไปทางผิวหนัง แต่จะเข้าไปในข้อ ส่งผลให้มีการอักเสบติดเชื้อของข้อ (Septic arthritis) ข้อที่พบว่ามีการติดเชื้อได้บ่อย คือ ข้อสะโพก ข้อศอก ข้อไหล่ ข้อเข่า ข้อมือ การตรวจทางภาพถ่ายรังสี พบว่ามีการบวมของเนื้อเยื่ออ่อนรอบๆ ข้อ กระดูกรอบๆ ข้อ บางลงใน 2 สัปดาห์แรกของการติดเชื้อ

Septic Arthritis หลัง 2 สัปดาห์อาจเห็นช่องข้อแคบลง กระดูกแหว่งเป็นหย่อมๆ ติดเชื้อแบคทีเรีย เชื่อที่พบได้บ่อยที่สุดคือ Staphylococcus aureus รองลงมาคือ Streptococcus เชื้อโรคเข้าได้ 3 ทางคือ กระแสเลือด บาดแผล การลุกลามจากบริเวณกระดูกที่อักเสบ

32 มี pannus เกิดขึ้น ทำให้กระดูกอ่อนไม่ได้รับอาหารจากน้ำหล่อเลี้ยงข้อ
Septic Arthritis พยาธิสภาพ เมื่อเกิดการติดเชื้อในข้อจะทำให้ synovium membrane บวม มีเลือดมาคั่ง และมีของเหลว (exudate) เข้าไปในข้อ ทำให้ข้อบวมตึง จากนั้นของเหลวในข้อจะขุ่นมากขึ้น จนกลายเป็นหนองและทำลายกระดูกอ่อนผิวข้อ มี pannus เกิดขึ้น ทำให้กระดูกอ่อนไม่ได้รับอาหารจากน้ำหล่อเลี้ยงข้อ เยื่อหุ้มข้อ (capsule) หนาตัวขึ้น ทำให้เคลื่อนไหวข้อได้น้อยลง เกิดข้อยึดติดกัน (fibrous ankylosing) หรือข้อเชื่อมกัน (bony ankylosis) ในที่สุด

Septic Arthritis Nursing care การรักษาทางการพยาบาล ดูแลให้ได้รับประทานอาหารที่มีโปรตีนสูง วิตามินซี และ High calories เนื่องจากร่างกายมีการติดเชื้อ ร่างกายจะเพิ่ม metabolism ขึ้น การให้สารอาหารพวกโปรตีน คาร์โบไฮเดรตจะช่วยให้พลังงานเพียงพอ สำหรับวิตามินซี จะช่วยเพิ่มคอลลาเจน ซึ่งเสริมสร้างการหายของบาดแผล ดูแลให้ผู้ป่วยออกกำลังกายข้อต่างๆ ทุกวัน และให้ผู้ป่วยมีกิจกรรมต่างๆ มากขึ้น หลังจากไข้ลด และข้อบวมยุบลงแล้ว
