Case report หญิงตั้งครรภ์ อายุ 30 ปี nullipara GA 40 weeks BW 60 kg Height 156 cm มาโรงพยาบาลด้วยอาการเจ็บครรภ์คลอด ประวัติการตั้งครรภ์ปกติดี ไม่มีโรคประจำตัว admitted ที่ห้องรอคลอดจนกระทั่ง fully dilated และถูกย้ายเข้าห้องคลอด แผนภูมิที่ 1 ประกอบ
ที่ห้องคลอดผู้ป่วยใช้เวลาเบ่งอยู่นาน เนื่องจากเด็กตัวค่อนข้างใหญ่ จนกระทั่งแม่ exhausted และ prolong second stage of labor สูติแพทย์จึงตัดสินใจใช้ Vacuum extraction ในที่สุดเด็กคลอด น้ำหนักตัว 3500 grams ไม่ได้ทำ AMTSL no AMTSL
Dep of Anesthesiology, KKU 1 ชั่วโมงถัดมา พยาบาลแจ้งแพทย์ผู้ทำคลอดว่า BP 70/40 mmHg PR 140 bpm และมีเลือดออกทาง Vg ค่อนข้างมาก จากการตรวจไม่พบ retain placenta or cervical rupture แต่พบว่ามี uterine atony and vaginal laceration with profuse bleeding ‘too little, too late’ July 17,2008 Dep of Anesthesiology, KKU

Emergency exploratory laparotomy massive amount of ascites and a large atonic uterus Bimanual compression, oxytocin intravenous infusion, and sulproston (250 mgx2) injection into the myometrium, the uterus remained atonic with profuse vaginal bleeding.
