การจ่ายย้อนหลังตามต้นทุนทั้งหมด ( Reimbursement at full cost )= Fee for service (Retrospective payment system) การจ่ายเงินตามยอดสมาชิกที่มีการขึ้นทะเบียน (Reimbursement per capita ) การจ่ายเงินตามการเจ็บป่วยจริงโดยมีการตกลงราคาค่าบริการล่วงหน้า (Prospective payment system based on DRGs)

6 DRG เป็นระบบการจัดกลุ่มผู้ป่วย แบบหนึ่ง
เพื่อให้ผู้ป่วยที่มีการใช้ทรัพยากร ในการรักษาใกล้เคียงกัน อยู่ในกลุ่มเดียวกัน ข้อมูลที่ใช้ในการจัดกลุ่ม คือการวินิจฉัย การรักษา ผลการรักษาและข้อมูลบุคคลของผป.

7 ข้อมูลที่ใช้ในการหา DRG
Patient’s Data : PDx, SDx, Proc, Age, Sex, Weight, D/C Type DRG Grouping

8 ปัจจุบันประเทศไทยใช้
ICD-10TM 2007 ของไทยให้รหัสโรค ICD-9-CM 2007 ของอเมริกาในการให้รหัสการผ่าตัดและหัตถการต่าง ๆ 1 ตค. 53 อาจประกาศใช้ DRG Ver 5.0 ใช้ ICD-10TM 2010, ICD-9CM 2010

9 โดยแบ่งเป็น 25 Major Diagnostic Category (MDC)
ระบบ DRG ใช้วิธีแบ่งผู้ป่วยออกเป็นกลุ่มย่อย ตามกายวิภาค (Anatomical system ) โดยแบ่งเป็น 25 Major Diagnostic Category (MDC) DRG Ver5.0 เพิ่ม MDC 28 (LOS <6 hr.)

ภาวะแทรกซ้อน หรือโรคอื่นที่เป็นร่วมด้วย ทําให้มีความยุ่งยากในการรักษามากขึ้น มีการใช้ทรัพยากรในการรักษามากขึ้น SDx => CC

21 SDx (Secondary Diagnosis)
- โรคอื่นๆ (ที่ active) เช่น โรคร่วม ภาวะแทรกซ้อน - ใช้รหัส ICD-10 - อาจมี 0 diagnosis หรือมากกว่า
