16 The possible potential scenarios of H5N1, H7N9, H9N2,H7N7, H2N2…….
The virus could simply disappear (unlikely) The virus could persist causing sporadic case, small cluster of cases (similar to H5N1 situation) The virus could evolve to pandemic spread eg. mutation or reassortment (unpredictable) Possibility of SUSTAINED H-to-H TRANSMISSION

17 Hong Kong Report Human H9N2 Avian Flu Case
H9N2 has been reported sporadically from HK in 1999, 2003, 2007, 2008, 2009 All were mild respiratory tract infection Dec. 28, 2013 :- reported a man with H9N2 from Shenzhen (first case in 4 yr.) :- No history of contact with live poultry, undercooked poultry or contact flu patient :- home contact has remained asymptomatic ( Dec 30, Hong Kong CHP news release. )

7-yr. old child from Yongzhou, Hunan province Fever + runny nose on Nov. 19, 2013 Recovered from mild RTI after Rx History of poultry contact ( Jan 2, Hong Kong CHP news release. )

19 China reports first human case of H10N8 Avian Flu Infection
73 yo. woman from Jiangxi province (NE of China) Immunocompromised illness had visited LPM Died from severe pneumonia on Dec 6, 2013 H10N8 had been detected in duck in Guangdong, China, 2012 H10N7 had been detected in human in Australia and Egyptian infant in ( CIDRAP News, Dec 17, )

22 Human Coronavirus ก่อโรคทางเดินหายใจและทางเดินอาหาร (ส่วนใหญ่อาการน้อย บางครั้งรุนแรงมาก) ยกเว้นเมื่อ ปี ค.ศ พบ SARS Coronavirus มี 3 genus ( เป็น enveloped RNA virus) - Alphacoranavirus :- พบในค้างคาว มนุษย์ สุกร - Betacoronavirus :- พบในหนู ค้างคาว - Gammacoronavirus :- พบในสัตว์ปีก หนู mammals

130 confirmed cases from 8 countries 58 (45%) has been fatal Median age is 50 yr. (range : 2-94 yrs.) Male : Female = 1.6 : 1 23 (18%) were HCWs 27 (21%) were milder asymptomatic cases Limited evidence of person-to-person transmission ( MMWR September 27, 2013/62(38): )

8 clusters of illness have been reported by 6 countries :– France, Italy, Jordan, KSA, Tunisia and UK Clear evidence of human-to-human transmission Largest cluster was in AI-Ahsa, KSA with 25 cases, death 14 associated health-care facilities, 2 HCWs ( .cdc.gov/HAN/han asp. )

25 Clinical Manifestations of MERS-CoV Infection
Asymptomatic - unusual (rumor in Italy, France) Mild symptomatic - rare, occur in children Pneumonia - common Severe pneumonia, ARDS - common Other :- GI symptoms, diarrhea, abdominal pain may be the presenting symptoms, ARF was common

26 Laboratory Investigation for MERS-CoV
PCR for MERS-COV is the gold standard Sample from Nasopharyngeal swabs is highly recommended - if patient on ventilator, lower RT specimens should be the priority including NP swab ( .cdc.gov/HAN/han asp. )

27 Treatment of MERS-CoV Infection
1. Supportive care :-Antipyretic ,Cough medicine, Rest ,Treat complications, Ventilator care,etc. 2. Antiviral agents eg. Ribavirin, etc.- (No effect) 3. Immunomodulators eg. Corticosteriod, IVIG, Convalescent serum, Interferon alpha, protease inhibitosr,etc. :– mostly no benefit or controversy

29 Personal Protective Equipment (PPE) for Healthcare Personnel (HCP)
• Gloves • Gowns • Eye protection (goggles or face shield) • Respiratory protection that is at least as protective as a fit-tested NIOSH-certified disposable N95 filtering facepiece respirator or N100 respirator
