4 แผน กสธ.ปีงบประมาณ 2558 แบ่งเป็น 3 Strategies ใช้เขตสุขภาพเป็นจุดบูรณาการ Age Group strategy.  ตัวหลัก สสจ. / สสอ / รพช / รพ.สต) Service plan Strategy. (1o , 2o , 3o )  ตัวหลัก รพศ/รพท Administration Strategy  ผู้ตรวจ ให้เขตทำแผนสุขภาพเขต โดย ให้เริ่มจากปัญหาของเรื่องนั้นๆ อย่าเริ่มที่ KPI หรือโครงการของกรมต่างๆ เมื่อได้ปัญหาให้ระบุมาตรการในการแก้ปัญหา เมื่อได้มาตรการ ถึงมากำหนดเป้าหมายหรือเครื่องชี้วัด เมื่อได้เป้าหมาย ถึงกำหนดแผนงาน/โครงการ/กิจกรรม และงบประมาณเพื่อให้สามารถบรรลุเป้าหมายนั้น จังหวัดทำแผนสุขภาพจังหวัด โดยใช้แผนเขตเป็นกรอบ อำเภอทำแผนสุขภาพอำเภอ โดยใช้แผนจังหวัดเป็นกรอบ (ไม่มีแผนตำบล)

9 การนำแผนสุขภาพเขตไปสู่การปฏิบัติ
ผู้ตรวจ และ สาธารณสุขนิเทศ เป็นผู้บูรณาการ Actors ต่างๆในระดับสาธารณสุข (Provider , Purchaser , Supporter) แต่งตั้ง Focal Point ในแผนต่างๆระดับเขต (5 Age group,3 Issues,and Service plan 10 สาขา) ในระดับจังหวัดมีการแต่งตั้ง PM (Project manager) Focal point ระดับจังหวัดเพื่อรับผิดชอบในแต่ละเรื่อง เช่น MCH manager, child manager ในระดับอำเภอ จะมีการตั้ง Focal point ระดับอำเภอเพื่อรับผิดชอบในแต่ละเรื่อง เช่น MCH manager, child manager ,Care manager
