2 ผู้ป่วยเพศหญิง อายุ 46 ปี

3 CC : ฟันกรามน้อยด้านบนที่อุดไปเสียวและมีเศษอาหารมาติด
HPI : ตอนแรกผู้ป่วยมีอาการเสียวฟันเมื่อดื่มน้ำเย็นและทานอาหารหวาน เมื่อหยุดทานอาการจะหายไปได้เอง มีอาการมา 2-3 เดือน จึงมาพบ นทพ. เพื่ออุดฟัน หลังจากอุดฟันไป 1 สัปดาห์ ผู้ป่วยมีอาการเสียวฟันมากกว่าเดิม

4 Effect of patient : ทำให้เคี้ยวอาหารได้ลำบาก
Patient’s belief : ยังอุดฟันได้ไม่แน่น จึงมีช่องว่าเหลืออยู่ทำให้เศษอาหารเข้าไปติดได้และเสียวฟัน

5 Past & Current Systemic Medical Conditions : รักษาอาการทางจิตเวช เนื่องจากมีความเครียด
Family history : No Psychosocial history & Environmental factor : ตอนนี้อาศัยอยู่กับสามี ซึ่งทำงานอยู่ที่ศูนย์ป้องกันสาธารณภัย ส่วนผู้ป่วยมีสวนยาง มีปัญหาด้านค่าใช้จ่ายเนื่องจากต้องส่งลูกเรียน 2 คน
