Case ผู้ป่วยหญิงตั้งครรภ์ไทย อายุ 29 ปี G2P1A0 , GA 11+2 weeks by LMP
Chief complaint : ปวดท้องด้านซ้ายล่าง 3 ชั่วโมงก่อนมาโรงพยาบาล 9 days PTA = GA 10 wks

Present illness : 2 สัปดาห์ก่อนมาโรงพยาบาล ประจำเดือนไม่มาสองเดือน จึงซื้อที่ทดสอบการตั้งครรภ์ด้วยตนเองมาตรวจ พบว่าเป็นผลบวก มีการตั้งครรภ์ 9 วันก่อนมาโรงพยาบาล ปวดท้องน้อยด้านซ้ายล่าง ปวดแบบบีบๆหน่วงๆ ปวดแต่ละครั้งนานประมาณ 2-3 ชั่วโมง ปวดเป็นๆหายๆ เมื่อนอนพักอาการดีขึ้น และมีเลือดออกทางช่องคลอด ปริมาณครึ่งแผ่นผ้าอนามัยต่อวัน มีเลือดออกอยู่ 2 วัน ไม่มีชิ้นเนื้อหลุดออกมา ไม่มีไข้ ไป ร.พ.นครนายก แพทย์ได้ทำการตรวจภายใน พบว่าปากมดลูกปิด อัลตราซาวด์ทางหน้าท้องไม่พบถุงการตั้งครรภ์ในมดลูก วินิจฉัยภาวะแท้งคุกคาม ให้กลับบ้านและนอนพัก 7 วันก่อนมาโรงพยาบาล ยังมีอาการปวดท้องด้านซ้ายล่างอยู่ ไม่มีเลือดออกทางช่องคลอด ไม่มีไข้ ไปพบแพทย์ที่คลินิก แพทย์ได้ทำอัลตราซาวด์ทางหน้าท้องไม่พบถุงการตั้งครรภ์ในมดลูก และพบว่าปีกมดลูกด้านซ้ายดูบวมขึ้นเล็กน้อย แพทย์ได้ให้ยาฆ่าเชื้อมากิน

Present illness : 2 วันก่อนมาโรงพยาบาล แพทย์คลินิกนัดมาติดตามอาการ ผู้ป่วยยังมีอาการปวดท้องอยู่ มีเลือดออกทางช่องคลอดสองวันก่อน ปริมาณครึ่งแผ่นอนามัย แต่วันนี้ไม่มีแล้ว ไม่มีชิ้นเนื้อหลุดออกมา แพทย์สงสัยการตั้งครรภ์นอกมดลูก แนะนำให้ผู้ป่วยไปร.พ. ผู้ป่วยไป ร.พ.นครนายก แพทย์ให้นอนสังเกตอาการ 1 คืนและให้น้ำเกลือ และให้กลับบ้าน นัดมาติดตามอาการอีก 1 สัปดาห์ 3 ชั่วโมงก่อนมาโรงพยาบาล ปวดท้องน้อยด้านซ้ายล่างมากขึ้น ปวดบีบๆหน่วงๆ มีอาเจียน 3 ครั้ง มีเวียนศีรษะ อ่อนเพลีย รับประทานอาหารได้น้อยลง ไม่มีประวัติกระทบกระแทก ไม่มีเลือดออกทางช่องคลอด ไม่มีชิ้นเนื้อหลุดออกมา ไม่มีไข้ ไม่มีตกขาวผิดปกติ ไม่มีปัสสาวะแสบขัดขุ่น

Past history : No underlying disease No food or drug allergy
No current medication No smoking/ no alcohol drinking Hx. Of surgery : C/S(2554)

OB-GYN history : LMP 14/1/58, duration 2 days, interval 2-3 months, amount 3-4 pads/day PMP กลางเดือน พ.ย. 2557 G2P1A0, GA 11+2 weeks by LMP ยังไม่ได้ฝากครรภ์ G1 (2554) : term, c/s due to unfavorable cervix with GDMA2 ที่ร.พ.นครนายก, ทารกเสียชีวิตหลังคลอด 3 วัน ด้วยภาวะเลือดออกในปอด No history of STDs or PID No multiple partners No Hx. Of PAP smear Last SI 2 weeks PTA

Physical examinations
Vital signs : BT 36.9 ⁰c, BP 130/85 mmHg, PR 104 bpm, RR 20/min GA : A Thai pregnancy, good consciousness HEENT : not pale conjunctivae, anicteric sclera CVS : normal S1&S2, no murmur RS : both lungs are clear Abd : soft, marked tender at LLQ, rebound tenderness, guarding, FH can’t be palpated Ext : no pitting edema, cap. Refill < 2 secs

Physical examinations
PV : MIUB – normal Vagina – old bloody discharge Cervix – os closed, minimal old blood stained, cervical motion tenderness Uterus – normal size, not tender Adnexa – tender at left adnexa, no mass palpable Cul-de-sac – bulging

Left tubal pregnancy at ampulla, size 3 cm. with active bleeding per left fimbriae Hemoperitoneum 700 ml with blood clot
