7 Total Contact Cast (TCC)
TCC is general viewed as the best standard for off-loading the neuropathic wound and it is the most effectiveness modality for wound’s treatment. Its effectiveness is around 90% within 5.5 weeks. The total-contact cast has proven to be the gold standard of treatment because of its ability to reduce pressure and facilitate patient adherence to the off-loading regimen

8 Indications Superficial wound (1A) Non infected
Neuropathic patients without peripheral arterial disease (PAD) /chronic lower extremity ischemia (CLEI) Deep fascia, muscles, tendons, joints, or bones but that are without evidence of infection

9 Contraindications Ischaemia
Deep foot ulcers where abscess, osteomyelitis, or similar deep infection or gangrene Highly exudative ulcers Severe fluctuating edema Patient compliance and unstable gait.

15 Advantages and Disadvantages of TCC

16 Removable cast/Aircast
Removable cast is an option for off-loading if patients are refuse for TCC but accept for this modalities. Incase of maceration/ exceed exudates and need to change dressing. Disadvantage of removable cast - It can take on and off, thus, patients sometime deny for put on it. - Next time, when patients put on it , the position could be change.

จากขอบของแผลประมาณ 5-10 มิลลิเมตร วิธีนี้ผู้ป่วยสามารถใส่รองเท้าติดไปกับเท้า ได้ ข้อดี วิธีนี้ผู้ป่วยสามารถเปิดทำแผลได้เอง ข้อด้อย โฟมสามารถเลื่อนตำแหน่งไปกดแผลได้ Felted foam is the compromise method for patients if they refuse for TCC or other methods.
