2 ชนิดของวิตามินดี วิตามินดีมี 2 ชนิดคือ
Ergocalciferol (vitamin D2) – เป็นวิตามินดีที่ได้จากพืช Cholecalciferol (vitamin D3) – เป็นวิตามินดีที่ได้จากสัตว์ หรือสังเคราะห์ขึ้นเองโดยอาศัยแสงอาทิตย์

3 แหล่งสำคัญของวิตามินดีได้มาจาก
1. อาหาร (น้ำมันตับปลา ปลาไหล ปลา salmon น้ำนมเสริมวิตามินดี) 2. ได้จากการสังเคราะห์วิตามินดีที่ผิวหนังโดยแสงแดด ส่วนใหญ่ของวิตามินดีที่มนุษย์ได้รับมาจากการสังเคราะห์ทางผิวหนังโดยแสงแดด

4 แหล่งอาหารที่มีวิตามินดีสูง
1. ปลาไหล มีปริมาณ vit. D 1,814 IU/1 oz. 2. น้ำมันตับปลา มี vit. D 400 IU/1 tsp. 3. ปลา Salmon มี vit. D 142 IU/1 oz. 4. ปลา Sardines มี vit. D 85 IU/1 oz.

8 ผลของการขาดวิตามินดี
หากขาดวิตามินดีจะทำให้แคลเซียมในกระแสเลือดลดลง เกิดภาวะ secondary hyperparathyroidism เพิ่มการสลายกระดูก โดยเฉพาะ cortical bone ทำให้มวลกระดูกลดลงนำไปสู่ภาวะของกระดูกพรุนในที่สุด กระดูกที่มักจะได้รับผลกระทบมากที่สุดในกรณีนี้คือ Femoral neck

ผู้สูงอายุที่อาศัยอยู่แต่ในบ้าน มักพบ ภาวะของวิตามินดีในกระแสเลือดต่ำ (Gloth FMIII. JAMA. 1995) หลายๆการศึกษาในแถบซีกโลกตะวันตกพบว่าความชุกของ ภาวะวิตามินดีในกระแสเลือดต่ำมีสัดส่วนสูงถึงร้อยละ (Omdahl JL. Am J Clin Nutri 1982; McKenna MJ. Am J Med 1992; Goldray D. J Am Geriatr Soc 1989.)

12 สภาวะของวิตามินดีในประเทศไทย
ประเทศไทยตั้งอยู่ในช่วงเส้นรุ้งที่ 4-20 องศาเหนือ ซึ่งเป็นบริเวณที่ได้รับแสงแดดจ้าทั้งปี แพทย์ส่วนใหญ่จึงเชื่อว่าคนไทยควรจะได้รับแสงแดดเพียงพอ ด้วยเหตุนี้จึงเชื่อว่าคนไทยไม่ควรจะขาดวิตามินดี การเสริมวิตามินดีไม่มีความจำเป็นในผู้ป่วยโรคกระดูกพรุน ความเชื่อนี้มีความเป็นจริงมากน้อยเพียงใด ?

ระดับของวิตามินดี ต่ำสุดคือสตรีก่อนหมดประจำเดือน อันดับสองและสามพอๆกันคือสตรีวัยทองและสตรีสูงอายุในเมือง สูงที่สุดคือสตรีสูงอายุในชนบท ระดับวิตามินดีที่ถือว่าเป็นระดับของการขาดคือค่า calcidiol level < 35 ng/ml (จำนวน case ทั้งหมด 322 ราย)

25 สรุปผลการศึกษาวิตามินดีของคนไทย
ความชุกของการขาดวิตามินดี สูงสุดคือสตรีก่อนหมดประจำเดือน (77.98%) อันดับสองคือสตรีสูงอายุในเมือง (65.4%) อันดับสามคือสตรีวัยทอง (60%) ต่ำที่สุดคือสตรีสูงอายุในชนบท (15.4%)

แนวโน้มการขาดวิตามินดีจะมีเพิ่มขึ้นอย่างต่อเนื่องในอนาคต ผู้สูงอายุในอนาคตจะเป็นโรคกระดูกพรุนเนื่องจากการขาดวิตามินดีสูงขึ้น หากไม่มีการปรับเปลี่ยนพฤติกรรมตั้งแต่ปัจจุบันนี้ อุบัติการณ์ของโรคกระดูกพรุนจะเพิ่มขึ้นอย่างมากในอนาคต

49 FOSAMAX PLUS® Improved Vitamin D Status at 15 Weeks
