They include formal sources such as surveillance systems monitoring occurrence of cases over time. Those are usually based either on clinical reports by hospitals, general practitioners, specialists, etc.) or upon laboratory data reported by peripheral or national reference laboratories. The timeliness of detection and report is a prerequisite for an efficient investigation and response. In fact, often, outbreaks are detected via rumours or direct calls from health practitioners who have observed what appears to them as being an unusual event (signal). Sometimes the information may come from the public or the media. At the international level, some systems (GPHIN), using web scanning, automatically review media information available worldwide on a daily basis. Whatever the source of the information, the first question will be to judge if the alert based on this information merits an intervention and if cases identified constitute an outbreak that needs to be investigated. เป็นการระบาด หรือไม่ ?

In considering whether the alert/information provided constitutes an outbreak, one should consider whether the reported cases are more than the expected cases, whether cases are clustered in time, place and person and whether this pose a concern from involved health care workers, school, institution or even the media. Finally, one must be aware of artefactual causes of increases or decreases in reported cases such as changes in local reporting practices, increased interest, new health personnel/ health clinic among others before declaring it as an outbreak.

There are many reasons to investigate an outbreak. The obvious is to try to stop the outbreak by undertaking a public health function. This may constitute a major challenge in case of a very severe disease such as Ebola, avian influenza (AI) or SARS. An investigation will also help us to improve our knowledge and understanding of the disease of interest (mode of transmission, risk factors). As a consequence it will allow preventing new disease episodes. Since additional and more detailed information is collected from cases and the source population an outbreak investigation may also be an opportunity to evaluate the performance of related surveillance activities. If no surveillance system is yet available for the disease of interest the investigation is also the opportunity to set up surveillance and to constitute the necessary human and logistical network. Outbreak investigations can also serve research purposes allowing for testing new laboratory techniques, identifying new germs, looking at unusual risk factors and identify hypotheses for clinical and therapeutic research. In addition every investigation is unique and, if properly conducted, will bring new insight on the disease. It is also the best learning by doing activity available to teach and practice epidemiological methods.

เพื่อพิสูจน์หา: สาเหตุ ตัวเชื้อที่ทำให้เกิดโรค วิธีการถ่ายทอดโรค แหล่งที่มาของเชื้อ สาเหตุ พาหะที่มีความเป็นไปได้ ประชากรที่เสี่ยงต่อการเกิดโรค ลักษณะประชากรที่มีปัจจัยเสี่ยงหรือปัจจัยป้องกัน Slide 16 Specific objectives of an investigation The first objective in an investigation is to identify the cause, the germ responsible for cases occurrence, and to detect the mode of disease transmission particularly searching if it is transmitted from person to person or if a common source (punctual in time or continuing) is involved. The investigation should allow for identifying the source of the agent involved (food, water, environment, animal reservoir, etc.). Sometimes it is important to identify potential carriers of the germs even if they are asymptomatic and to precise their role in disease transmission and sustainability of infection in the population of interest. Also important is to define the source population for the cases, i.e. the population at risk of developing the disease. This will be the denominator of interest for any type of epidemiological study we may want to conduct during the investigation. Finally the identification of population characteristics that enhance or decrease disease transmission (risk or protective factors) will be needed in order to better adjust the intervention.

It is rare to be involved very early in the course of an outbreak However, with modern communication means, real time investigation has become possible (e.g. SARS). Most of the time cases have already been detected for several days or weeks. Most of the information collected will rely on quality of documents and patients memory. The investigation is done in real life condition and is therefore subject to many biases. The purpose of a field investigation is to draw sensible evidence from imperfect data. Sometimes data have already been collected and the task of the epidemiologist is to critically appraise the available information. Delays between disease or outbreak occurrence and alert may be long. This should however not be a reason not to investigate. Even if an event is out of media sight it remains a public health problem of interest which merits clarification and understanding. It is never to late, but it can also be more difficult

27 To Investigate or not to Investigate

Potential outbreaks may turn out to be true outbreaks with a common cause, or may be unrelated cases of the same disease. “By chance” there could have been a number of unrelated cases at the same time. Other factors in considering whether or not an investigation should be undertaken are the severity of the illness, its potential as a threat to the health of the public (how transmissible it is), and possibly local politics, public concern, or resources available to investigate.

A case definition is unique for every outbreak situation, but is always based on objective measures (4).

34 นิยามผู้ป่วย Sensitivity: Most cases detected, but …
many false positives many specimens to test low % tested specimens +ve SENSITIVITY SPECIFICITY Overload A case definition can emphasize getting all possible cases (sensitivity), or can emphasize having only the exact illness you are investigating as cases (specificity). Generally, you start with a “loose” definition early in the investigation, which lends itself to identifying anyone who might possibly be a case (this is a sensitive case definition). It is better to gather too much information than too little. For instance, getting information about patients that later end up not being true cases is better than having to go back and find cases that you mistakenly ruled out early on. And gathering information from potential cases about many exposures that turn out not to be related to the outbreak is easier than going back, trying to contact cases again, and asking more questions about exposures you did not include in your early investigation because of a narrow case definition. As you become more sure of the symptoms and the agent, the place of exposure, and the time frame, you can safely narrow the case definition. It may involve examining county or state surveillance data to identify cases reported through the communicable disease reporting system. In an outbreak situation, one may identify some cases through passive case finding, but it is imperative to perform active case finding. Multiple sources should be used to find cases, and the best methods may require some creativity on the part of the investigator.

This information depends on the nature of the outbreak. For example, if it pertained to an outbreak of diarrheal disease among preschool aged children it would be important to ask whether or not the child attended day care and, if so, the name of the facility. Demographic information also enables you to describe the at-risk group(s) of individuals. Clinical information such as clinical symptoms, the date of symptom onset, lab findings and severity of illness is important to collect. This information allows the investigator to verify that the case definition has been met, to characterize the disease and to create an epidemic curve.

46 Line Listing: What and Why?
เป็นการจัดระเบียบข้อมูลผู้ป่วย ทำให้ข้อมูลดูง่าย ทบทวนความถูกต้องและแก้ไขได้ง่าย ตาราง แถว หมายถึง ผู้ป่วยแต่ละราย คอลัมน์ หมายถึง ตัวแปรแต่ละตัวที่สนใจ Using the information described, a line listing is produced by epidemiologists in outbreak investigations. A line listing allows information regarding time, person, and place to be organized and reviewed quickly. It is also a good way to keep track of different categories of cases. For example, cases can be entered into the line listing as possible, probable or confirmed (laboratory-confirmed, clinically-confirmed or both). Example: In a 1997 outbreak of E. coli 0157:H7 over 50% of the cases occurred in women. This was quite different from other E. coli 0157:H7 outbreaks found to be associated with beef products. In this outbreak, the surprising proportion of female cases probably clued the investigators into the possibility of a non-hamburger reservoir, which was found to be alfalfa sprouts (7). Frequency distributions of potential exposures, such as occupation, sexual behavior or recreational activities or hobbies may clue the investigators in to the outbreak source or transmission route. Example: In an outbreak of multi-drug resistant Salmonella typhimurium, cases were employees of a veterinary facility. Although most cases of Salmonella infections in the U.S. are transmitted by food, they can also be transmitted through farm animals, reptiles and pets (8). Spot maps/Geographic Information Systems (GIS) can be used to plot locations such as residence or place of employment. This information may provide clues about potential exposure patterns in the outbreak. If the overall population varies between areas on the map, the investigator should plot the attack rate in each area (instead of number of cases) because only plotting the number of cases can be misleading. Example: In an investigation of nosocomial surgical site infections in a large medical facility, a spot map of the hospital may show clustering of the cases by operating room.

If there is no obvious connection among the cases, making the cohort difficult to identify, or the cohort is too large to contact all members, then a case-control study may be the best choice. The most difficult part of a case-control study is selecting the controls. Defining the source population (the population that gave rise to the cases) may help to narrow down potential control selection. Do the cases live in the same city, or did they attend the same event? Are they of a particular race or ethnicity? Understanding where the cases came from will help you select your controls.

Human clinical specimens include blood, serum, urine, saliva, hair, feces, etc. The type of specimen collected depends on the nature of the outbreak. For example, stool specimens may be collected during an outbreak of diarrheal illness, while blood may be collected during an outbreak of fever and rash suggestive of rubella. Similar specimens can also be taken from animals when investigators suspect that they may be involved in an outbreak.

It is always preferable to implement these measures in a voluntary manner, and WHO recommends using the least restrictive measures possible. Isolation is the separation and restricted movement of ill persons with contagious disease. A multidisciplinary team can ensure the coverage of different areas of expertise at the same time and thus joint experts and experiences that can enhance the identification of source. The descriptive analysis is the first step to find the solution. Thus good data collection and data descriptions are prerequisite for good results of the investigation.
